What Is NAD+ and Why Does It Matter?
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every cell in your body. Your cells use it in the processes that turn food into usable energy, and it also plays a role in cellular repair.
Natural NAD+ levels are generally understood to decline as we age. That is a big reason NAD+ has become a popular topic in healthy aging and performance circles. NAD+ therapy is designed to support your cells by delivering NAD+ directly, either through a vein (IV) or into the muscle (IM shot).

What Is Niagen?
Niagen is a brand name for nicotinamide riboside (NR), a form of vitamin B3. NR is a NAD+ precursor. That means it is a building block your body can convert into NAD+ on its own.
Think of it this way. NAD+ therapy hands your body the finished product. Niagen hands your body the raw material and lets your cells do the conversion. Both approaches aim to support healthy NAD+ levels, just by different routes.
Niagen is best known as an oral supplement. At Onus, it is also available as an IV drip and as an IM shot, so you can receive it in a clinical setting.

Direct delivery vs. the precursor pathway
With NAD+ therapy, the NAD+ goes straight into your system. With Niagen, your body does the work of converting the precursor. Neither approach is "better" in every case. They are simply different paths, and the right one depends on what you want and how you like to receive it.
One note on doses. The milligram amounts for NAD+ and Niagen are not a direct one-to-one comparison, because they are different compounds. Do not assume 250mg of one equals 250mg of the other. Our clinical team can talk you through dosing.
Time in the chair and how it feels
This is where the two options differ most.
NAD+ IV therapy can take 2 to 4 hours. At Onus, some clients tolerate infusions in as little as 45 minutes, and the average is around 2.5 hours. The pace depends on how your body handles the infusion, and our nurses adjust as needed. Bring a book, recline in one of our comfy recliners, and settle in.
Niagen IV therapy typically takes about 30 to 60 minutes, depending on dose and how well the infusion is tolerated. Some clients notice a slight tingling sensation on their lips and extremities. It fades when the infusion ends.
If your schedule is tight, the shorter Niagen session may be the deciding factor.
Niagen IV vs NAD+ IV: Which Should You Choose?
There is no single winner. Here is a simple way to think it through, based on your goals. These are general guidelines, not medical advice, and results vary from person to person.
- You want to support cognitive clarity and daily energy. NAD+ IV is a popular choice for people focused on mental sharpness and cellular energy. Plan on a longer visit.
- You are short on time. Niagen IV is designed for a much shorter session, often 30 to 60 minutes.
- You are a first-timer. Starting with a lower dose, or a quick IM shot, lets you see how you feel before committing to a longer infusion.
- You train hard and want to support recovery. Many endurance athletes pair NAD+ or Niagen with other recovery-focused options. Ask our team what fits your training load.
- You want ongoing maintenance. Members often choose a regular schedule of IM shots or IV sessions, since member pricing makes repeat visits more affordable.

IV vs IM: Which Delivery Format Fits You?
Both NAD+ and Niagen come in two formats.
IV drips deliver the dose through a vein over a set period. They allow higher dose options and are the go-to for people who want a more thorough session.
IM shots are given into the muscle and are much quicker. A NAD+ shot takes about 15 to 20 minutes. Most clients report a slight rising sensation that fades within minutes, though individual experience varies. A Niagen shot takes about 20 minutes, and some clients notice a mild tingling on the lips and extremities that fades with time.
IM shots are a good fit if you want convenience, a lower price point, or an easy way to try NAD+ or Niagen for the first time.

How membership changes the math
The Onus Memberships run $105 to $150 per month. Member pricing on these treatments adds up quickly. For example, a 250mg NAD+ IV drops from $275 to $195, a savings of $80 per visit. A 250mg Niagen IV drops from $425 to $297.50, a savings of $127.50 per visit. If you plan to come in regularly, membership is worth a look.
You can see every option on our NAD and Niagen menu.
A word on oral supplements
Oral NR and NMN supplements cost less than IV or IM treatments, and some people choose them for daily use. IV and IM therapy bypass the digestive system and are delivered in a clinical setting, which is why many people choose them. We think you should weigh both options. If you are unsure, ask our team and we will give you an honest take.
What Should You Know About Safety and Who Delivers Your Treatment?
Every Onus treatment is delivered by ER- and ICU-certified registered nurses, paramedics, and EMTs following hospital-based protocols. Our team works under the medical direction of Dr. Benjamin Wilks, MD. We hold a high clinical standard because vascular access and patient monitoring matter, especially for longer infusions.
Before any NAD+ or Niagen treatment, talk with a provider if you are pregnant, have a health condition, or take medications. Our team will review your history and answer your questions. These treatments are designed to support wellness. They are not intended to diagnose, treat, cure, or prevent any disease, and individual results vary.
